Application Forms

During your job hunt, you will probably be asked to fill out many application forms. These forms are not as complicated as they look. They simply ask questions about you and your qualifications.

The sample application form included in this package can be used to your benefit. Fill it out and take it with you to interviews to use as a guide when filling out real application forms.

Tips when Completing Application Forms

    • Read the entire application form before filling it out.
    • Read the questions carefully and answer the questions accordingly.
    • Print very neatly and legibly.
    • Make sure your application form has no spelling or grammatical errors.
    • Never write "please see résumé" as an answer to a question.
    • Provide answers that emphasize your skills and abilities.
    • If you are asked why you left your last job, give a neutral reason.
    • Attach your résumé to the application.

Ontario's Human Rights Code clearly states that some questions must not be asked on an application form. This includes questions about "race, ancestry, place of origin, colour, ethnic origin, citizenship, creed, sex, sexual orientation, age, record of offences, marital status, family status and handicap" [Section 23 (2)]. The term "handicap" is described in the Code as "any degree of physical disability, infirmity, malformation or disfigurement that is caused by bodily injury, birth defect or illness… including diabetes mellitus, epilepsy, any degree of paralysis, amputation, lack of physical co-ordination, blindness or visual impediment, deafness or hearing impediment, muteness or speech impediment, or physical reliance on a guide dog or on a wheelchair or other remedial appliance or device". [Section 10 (1) a]

If you find an inappropriate question on an application form, you can draw a line through the space provided or write "Not Applicable". Do not leave any empty spaces on your application form: the employer may assume you are careless.
