Community Care Access Centres
What are Community Care Access Centres (CCACs)?
CCACs are organizations based in your community that are there to assist you in making informed choices about managing your own health care needs, or of those that you care about.
The CCAC helps to coordinate:
1. Health care services provided in your own home, such as relief or nursing care.
2. Admission to long term facilities like nursing homes and seniors' residences.
3. Access to educational support services for children with special needs.
The CCAC can also provide you with information about other support organizations available within your community.
Anyone can contact the CCAC in your community (see below) for an assessment to determine eligibility for services – whether in your home or help for children at school. The CCAC will coordinate services to meet a variety of needs including nursing, various therapies, help with nutritional needs and personal support.
CCACs purchase services directly from the service provider on behalf of the client and all services are fully funded by the Ministry of Health and Long Term Care.
